Puppy Questionaire

The following is a questionaire I like to use to provide me with information on the prospective buyer, so I can get to know you better and also let you specify what your preferences are. Some of the personal information will be needed if you plan to have a puppy shipped (i.e. two phone numbers, address, etc) Please take some time to complete this form and it will be emailed to me if you are seriously interested in getting one of our puppies. Please note that there is no wrong answers to these. If you are honest with me we can match the right puppy to you.

Once the puppies are“on the ground” I will post the information on the website. Puppy picking order will be by deposit received. I also try to update the pictures at least once a week, I usually do this on Sundays. Please feel free to call or write anytime, as it helps me to know you are motivated. Thank you for your time.
